Clinical screening
Screening determines whether evaluation is appropriate. Submitting information or completing screening does not establish a patient-provider relationship.
Before any appointment can be booked we are required to hold your legal name, date of birth, phone, home address, an emergency contact or support person, and your primary-care provider’s details. Before each telehealth session we confirm your current physical location (state and address) and a callback number.
